Players Refusing to be subbed

The view of it I had was that the keeper went down. The manager thought he was injured therefore wanted to sub him. The keeper only had cramp so when he got up he wasn't saying no I'm not going off he was signaling to say that no he wasnt injured and was OK to carry on. Now being the number 1 keeper he would assume that the manager would want him to carry on if he was OK as he is his number 1 keeper. I may be cutting him to much slack but whatever the situation was it wasn't handled at all well by the medical staff, keeper or management.
